The chilling scene shows Wendy Rush, a director at Oceangate alongside her husband, sitting on the support ship when she hears a loud noise and cries out, "What was that bang?" Soon after, she receives a text claiming the sub had dropped weights, leading her to wrongly believe the dive was still on course.

Unfortunately, what Mrs. Rush heard was actually the sound of Titan's hull collapsing. The investigation has revealed that the carbon fiber structure of the sub was flawed and had been deteriorating since a previous dive a year prior. Experts, including some former Oceangate staff, had raised concerns about the design, warning that the Titan was unsafe for deep-sea exploration. The USCG confirmed that the sub had never undergone independent safety evaluations, showcasing how risks went unaddressed.
